If, after reviewing the benefits of membership and standards of practice, you are interested in being listed in the wellpdx practitioner directory, please fill out our online application, make your payment of the $100 annual membership due via paypal or a check, and mail a business card and any other marketing materials you would like us to have to: wellpdx / PO Box 1792/ Portland, OR / 97207

A wellpdx practitioner membership is $100/year
We ask for payment before processing your application as a gesture of good faith. If you application is not accepted, you will be fully refunded.

Benefits of Membership:

  • Listing in the wellpdx online directory, searchable by practice name, location, modality, and specialty.
  • The opportunity to network and get to know other members of the Portland wellness community, both online and at wellpdx sponsorerd events.
  • Invitation to participate in quarterly practice-specific and interdisciplinary round tables.
  • The fun of being part of something local and home-grown!

wellpdx Standards of Practice:

  • Responsible to an Oregon State Board and in good standing with that board
    We verify all information regarding licensure and certification either during the office visit or by contacting the appropriate board.
  • Hold good standing in professional community
    We ask for and verify a minimum of two peer references.
  • Is an individual practitioner
    wellpdx lists individual practitioners, not clinics or larger businesses.
  • Provides a safe, clean, and respectable environment for patients
    Because wellpdx is focused on community, an office visit is a part of the application process. This allows us to see where you work, and to meet you in person!
